Duty-Free Clearance on Destination Country Government Letterhead, 2. English Translation of Duty-Free Clearance, 3. Detailed Packing List in Required Format [provide link to packing list] 4. Letter from Valid Health Organization stating they need the medical items, can properly use them and can properly store them (if sending medical items), 5. Copy of packing list initialed by Valid Health Organization (if sending medical items) Donor may reapply once all documentation is in order. |
